Survey serves as ‘wake-up call’ to physicians regarding chronic pain management

A recent online survey of 2,063 US respondents found younger adults were more likely to experience chronic pain than older adults.

In particular, the survey conducted by The Harris Poll on behalf of the Samueli Foundation found that 65% of respondents ages 18 to 34 and 52% of respondents ages 35 and older had chronic pain.

Other survey results showed barriers to managing chronic pain: 75% of adults aged 18 to 34 said they did not know “what type of health care provider can best help them manage their pain”.

In addition, the survey found:

  • 32% of adults ages 18 to 34 reported back pain, making them the most commonly reported pain report, followed by 20% who reported either neck or knee pain;
  • Since the pandemic began, 29% of those aged 44 and younger have been speaking more often to their doctors about their pain, compared with 15% of those aged 45 and older; and
  • 22% of those aged 44 and younger use cannabis or CBD oil to treat their pain, compared with 11% of those aged 45 and older.

“It is clear that young people are trying to manage their chronic pain on their own, but they also want and need the help of their providers in determining the most effective treatments for their pain.” Wayne Jonas, MD, Executive director for inclusive health programs at the Samueli Foundation and former director of the NIH Office of Alternative Medicine, said in a press release. “GPs, who treat most pain sufferers, should guide their patients – especially young adults – to proven, effective strategies for managing their everyday pain.”

According to additional survey results from respondents who said they had chronic pain:

  • 78% reported using non-pharmacological therapies, while 70% reported using pharmacological therapies to relieve their chronic pain;
  • 80% of those who took non-pharmacological therapy said they would be interested in following a healthier eating plan to relieve their pain, while 71% were interested in trying out exercise and 68% were interested in massage therapy , 62% were interested in physical therapy and 61% were interested in mindfulness-based stress reduction or meditation; and
  • 53% reported using over-the-counter treatment to relieve their chronic pain, followed by 43% who reported exercising, 34% reported using heat and / or ice, and 26% reported being healthier feed, 16% who used cannabis or CBD, 15% who used either physiotherapy or massage therapy, and 14% who practiced yoga.

“This should be a wake-up call for doctors that their patients are looking for more information from them about the management of their chronic pain, especially with non-drug approaches,” Jonas said in the press release. “It is up to providers across the healthcare system to have regular discussions with patients to uncover the best options for treating their pain on a daily basis.”

In an interview with Healio Primary Care, Jonas discussed the survey results, current findings on the use of CBD and cannabis for chronic pain, when a patient should be referred to a specialist and more.

Healio basic care: Why do you think younger people are more likely to report chronic pain than older people?

Jonas: This result came as a bit of a surprise as we usually think that older people have more arthritis and chronic health problems.

Chronic pain is persistent pain felt for 12 weeks or more due to trauma, stress, stress, and irritation to the body, as well as, in some situations, the lingering effect of an incident that triggered acute pain. Jonas: Currently, the best evidence suggests that cannabis and CBD oil may be most useful for neuropathic pain that is sometimes associated with diabetes and chemotherapy treatments.

However, most chronic pain in young people is of musculoskeletal origin, and there is insufficient evidence to support the use of CBD and cannabis for this purpose. Hence, the people who use CBD for this purpose may be using it inappropriately.

Healio Primary Care: What Should Doctors Say About Cannabis Use Disorder To Patients Using Cannabis Or CBD Oil?

Jonas: Any time a patient uses something like this repeatedly, there is a risk of side effects such as addiction. Using these products for chronic pain is like trying to put a patch on something that requires deeper investigation and treatment. It does suggest, however, that doctors should view cannabis as an attempt at self-treatment; these patients try to take pain management into their own hands. We should acknowledge this and look for ways to help them manage their pain appropriately.

Healio basic care: What complementary and alternative medical options can PCPs recommend to patients with chronic pain?

Jonas: There are several non-drug or complementary and integrative medicine approaches to chronic pain, and we have pretty good research showing that they are safe and effective.

The American College of Physicians has shown that therapeutic yoga can be very effective for back pain, as can massage and mind-body techniques. These would be the treatment options I would recommend doctors to guide and help their patients.

Healio basic care: What is a PCP?s role in chronic pain management? When should patients be referred to a pain specialist?

PCPs are the most likely providers to help patients manage their chronic pain. But pain associated with loss of bowel or bladder function requires immediate specialist care; Therefore, patients who experience this should be referred quickly for a specialized examination. The other red flag for referral to a specialist is pain that has lasted more than 3 months and that has not benefited from nonsteroidal or non-drug approaches, even if it does not result in loss of bowel or bladder function.

Jonas: Very few physicians have received training in non-drug, complementary, and integrative approaches to chronic pain, and some may not know how to access or receive reimbursement for such training.
