A Guide to Cannabidiol (CBD) Safety for Health Care Professionals



Cannabidiol (CBD) is a non-intoxicating plant cannabinoid or phytocannabinoid found in hemp (Cannabis sativa L with a THC limit of less than 0.3%). CBD has a variety of molecular targets that are yet to be elucidated, but they are known to include indirect activities at cannabinoid receptors within the endocannabinoid system (ECS) as well as targets outside the ECS such as TRPV and 5-HT1A receptor sites. 1

Given the widespread use of CBD and its multitude of molecular targets, it is important that clinicians understand the potential for drug interactions with prescription or over-the-counter drugs.

Pharmacokinetic and pharmacodynamic interactions

Pharmacokinetics describes interactions that ultimately affect the serum levels of a drug and result in changes in drug concentrations at the site of action. The potential for pharmacokinetic interactions increases when substances that are ingested together share routes of absorption, distribution, metabolism, or excretion.

Pharmacodynamics describes interactions that affect the mechanism of action of a drug. In this situation, a substance that is co-administered with a drug can counteract or increase the clinical effect of that drug without changing the serum concentration. Data documenting these types of interactions for CBD are currently limited. 2

Most clinical trials have looked at Epidiolex®, a CBD isolate approved by the FDA for the treatment of refractory epilepsy. CBD isolates are obviously different from full-spectrum hemp extracts and are used in atypical doses; for example between 25-1,500 mg per day, with most studies being in the 300-600 mg per day range.

Cytochrome P450 metabolizing enzymes

The cytochromes P450 (CYPs) are a super system of enzymes that are mainly found in the liver, but also in the kidneys, gastrointestinal tract, lungs and skin. Most pharmaceutical drugs undergo phase I metabolism by hepatic CYP isozymes, followed by phase II conjugation reactions catalyzed by a hepatic UDP-glucuronosyltransferase (UGT) enzyme 4,5

Some nutrients, foods, and herbs are also metabolized by CYPs and, if taken together, can have deleterious effects on drug metabolism. For example, various chemicals in grapefruit can inhibit the CYP3A4 isozyme in the gastrointestinal tract, thereby increasing the bioavailability of certain oral drugs, potentially leading to adverse drug reactions

Overview of CBD Pharmacokinetics

The pharmacokinetics of CBD are complex and depend on the formulation and route of administration. Cannabinoids are generally highly lipophilic.

Ingestible oil-based CBD formulations have low water solubility and unpredictable absorption from the gastrointestinal system, resulting in variable pharmacokinetics (Devinsky et al., 2014). Studies with CBD in humans have shown that Cmax (measure of absorption, i.e. maximum plasma concentration measured over a period of time) is dose-dependent and Tmax (time to reach the measured maximum plasma concentration) generally occurs within 1-4 hours 1. The mean half-life of CBD (last time until plasma concentration halved) is 1.09 hours after a single oral dose of 10 mg, 1.97 hours after a 20 mg dose, and between 2-5 days after chronic oral ingestion

Admission – The absorption influences the onset of the pharmacological or physiological effect. CBD is highly lipophilic and has a low bioavailability after ingestion. Oral bioavailability is estimated to be only 6% due to the extensive first-pass metabolism in the liver, but Cmax is increased 4 to 5-fold when CBD is ingested with food, especially a high-fat meal. 11

Distribution – The distribution influences the duration of action and is influenced by the protein binding, ie the extent to which a substance is present in the circulation in connection with plasma proteins. CBD is highly protein bound, with about 10% bound to circulating red blood cells.

Metabolism – CBD is subject to extensive hepatic metabolism through CYP1A1, CYP1A2, CYP2C9, CYP2C19, CYP2D6, CYP3A4 and CYP3A5, but mainly through CYP3A4, CYP2C9 and CYP2C19, three isozymes that are responsible for the total liver activity of between 20 and 14% of the main metabolite of P450 CBD, 7-hydroxycannabidiol (7-OH-CBD), is then further metabolized into up to 100 metabolites, which are then excreted in the stool and urine. 15

Elimination – A study looking at repeated daily oral administration of CBD found that the elimination half-life was between 2 and 5 days. 16

Possible interactions

Both in vitro and animal studies show that CBD can inhibit P450 isozymes, particularly classes CYP2C and CYP3A17, but the clinical relevance of many of these interactions is not always apparent. 18

Of nearly 800 studies in a 2018 systematic review of CBD pharmacokinetics in humans, only data from 24 were deemed appropriate for quantitative comparisons and conclusions to be drawn.8 Clinicians should also be aware that both CYP2C9 and CYP2C19 are profound are polymorphic and are both overactive and underactive genetic variants.19 Individuals with these polymorphisms may have an increased likelihood of experiencing cannabinoid-drug interactions.

CBD in the form of Epidiolex has been shown to increase N-desmethylclobazam levels by inducing CYP3A4 activity and inhibiting CYP2C19 activity.21 A clinically significant interaction has also been observed between CBD in the form of Epidiolex and the widely used oral Anticoagulant warfarin observed. 22

Liver load and safety profile

The pharmacokinetics and safety of a single oral dose of 200 mg CBD in the form of Epidiolex were investigated in a small study in people with mild to severe hepatic impairment. Moderate and severe liver dysfunction decreased biotransformation, resulting in increased exposure to CBD: 2.5 times the exposure in those with moderate hepatic impairment and five times the exposure in those with severe impairment compared to those with normal liver function.

The 200 mg dose of CBD used in this study was found to be safe and well tolerated by all subjects, but the researchers recommended a lower starting dose and slower titration for those with moderate or severe hepatic impairment. 23

A 2011 review of the safety and side effects of CBD found that repeated use and high levels of up to 1,500 mg / day appear to be well tolerated in humans


Since CBD is subject to extensive hepatic metabolism, there is the potential for pharmacokinetic drug interactions that involve inhibition or induction of enzymes or transporters, similar to the CBD ‐ mediated inhibition of clobazam metabolism.

A basic guideline for clinicians is that if a prescription or over-the-counter drug has a “grapefruit” warning on the label, it can be assumed that there may be a potential interaction with CBD.

It is possible that full-spectrum hemp extracts have a more balanced pharmacokinetic and pharmacodynamic profile than the CBD isolates commonly used in CBD research.
